The Diabetes-PATH(Personal Action Towards Health) Workshop is Michigan’s version of the award winning Standford Chronic Disease Self-management program.  It is a six week workshop designed to provide the skills and tools needed by people living with type 2 Diabetes to improve their health and manage their symptomms.  You will be better equipped to face the daily challenge of living with diabetes.  In the workshop, you will ake a weekly action plan, share experiences, and help each other solve the problems faced by living with diabetes.

  3. Physical Activity

  5. Monitoring Blood Sugar

Diabetes PATH – Turner Senior Resuorce Center: October 1, 2013 (six weeks)
